"True Soldato" Achievement Doesn't Unlock

The "True Soldato" achievement/trophy is meant to be awarded after completing the game on Hard difficulty.
We've received reports from players that completing the game on Hard did not unlock this achievement. After testing, we believe this is caused by swapping the Difficulty in the Gameplay settings menu. If a player changes the difficulty even by accident, it may invalidate the progress towards this achievement on the current playthrough.
There is no workaround, or way to check the achievement progress, at this time. We are continuing to investigate.

This issue has been resolved after the most recent patch!
It is not retroactive and will only apply to playthroughs started after the update.
